Tumbleweed nursing home cited for neglect and control issues
Victim's granddaughter cites ongoing abuse after transfers from a prior facility. Tumbleweed, now at 1101 East Lake Street, faced DHS findings of policy gaps and substandard care. Family says records access was blocked and the patient regressed cognitively after the move. DHS had not opened a new investigation as of Friday afternoon.
