To qualify, a claimant must meet four general requirements including permanent disability and specific work history. The Social Security Administration will assess severity with medical evidence and guides, but full guides remain unreleased. Applications begin with a medical history form to be completed by a doctor and sent to a state agency for disability determinations, then reviewed by Social Security headquarters. If denied, a reconsideration or referee review is possible and can include new evidence. Next: how disabled you must be.
