House investigators say a fatal explosion occurred when oil flowed through a pumping unit near Fairbanks during repairs. Maintenance workers failed to secure a hatch and follow procedures, allowing oil to spray the building and ignite. One dead, five injured, cost of damage estimated at 20 to 50 million dollars.
