More than three million people yearly request SSA-7004 forms. The SSA shows how to obtain the Form SSA-7004 online via www.ssa.gov or by phone at 1-800-772-1213. Those with internet can download SSA-7004.PDF, print it, complete it, and mail it to SSA for a four to six week estimate. If over 60 and still employed, a mailed Personal Earnings and Benefit Statement arrives later this year, by September 30, 1995.
About 3.7 million children receive Social Security payments as survivors or dependents of retired or disabled workers. Benefits total over $1 billion monthly, with eligibility extending to biological, adopted, step, and certain grandchild relations. Payments can continue until marriage, age 18, 19 if in school, or longer if disabled.
