John E. Forsell of 310 Bellevue avenue North, a window cleaner for the Smith building manager, reportedly was fatally injured after slipping on the third floor and falling three stories at 12 16 p m Friday. He landed on his head and shoulders near pedestrian Peter Anderson who was on Second Avenue. Forsell was rushed to the city hospital.
