Deficiencies cited after Death Row escape in Ohio

A report found gross deficiencies in how Ohio State inmates on Death Row were monitored during a Feb. 3 escape attempt by Richard Covey and Maxwell White. The pair built a 13.4 foot ladder from bedsheets and magazines, reached an inner perimeter, and were caught after alarms sounded. They hoarded candy bars, water, matches, and a homemade fence-breaking tool. Guards were tipped but not informed to deputy wardens or the warden, enabling Covey to gain access through his unit.
