Mining disaster highlights flawed safety systems and delayed alerts
The report notes no maps for secondary escape routes and only about half the workers had carbon monoxide masks, many ineffective. An elevator operator may have died first, cutting off an escape path. Operators lacked two hours of oxygen per federal rules, and no general mine alarm existed. The company said the fire could have started days before detection May 2 and warned miners may have died hours or days earlier. The primitive telephone system at the shaft hubs hindered rescue efforts and misled miners about safe routes.
