Blue Cross and Blue Shield present non group coverage for individuals 19 or older and dependents under 19. The plan includes hospital care days starting at 21 per hospitalization, increases three days per year to a max of 30, plus maternity benefits after eight months. No health statement or exam required.
Blue Cross offers a Co-Pay Endorsement to the Non-Group Special Agreement. Under the lesser-benefit plan, subscribers pay 15 dollars per day for the first five hospital inpatient days, including maternity admissions, replacing the prior 5 dollar per day maternity provision. Other benefits stay the same and applications cover semiprivate or ward accommodations.
Blue Shield offers non group 100 protection with doctor identification and coverage for surgery oral surgery obstetrical services anesthesia and more. Enrollment must be postmarked by February 10, 1971 with a money order or check.
