Mercy Medical Center Cedar Rapids notified 97,000 patients that a third-party vendor, Perry Johnson & Associates, suffered a breach exposing names, birth dates, addresses, Social Security numbers, and medical record data. The incident, tied to 2011 2014 services, prompted free credit monitoring and a formal inquiry by Mercy.
