Blame Fixed for Bennington Disaster

Washington reports blame cast on Ensign Charlie T Wade who led the engineering department aboard the Bennington for approving subordinate valve reports and not ensuring safety valves worked. The court found the explosion caused by valve closure between the boiler and steam gauge, with several boilers damaged and the vessel aged. The inquiry notes the Bennington was in good discipline and condition aside from boilers, and cites the crew actions leading to the tragedy.
