In Clarke County Virginia jail the sheriff says over 50 toxic mattresses were replaced in a year amid a Tennessee fire that killed 42 inmates and prompted a probe. Officials note padded-cell foam fumes and toxic smoke caused the catastrophe and key mishaps hampered rescue efforts.
