It says the accident was caused by the failure of the crew of No. 151 to obey the train order at Bryn Athyn requiring the train to sit and wait until No. 156 had passed. The crew are described as experienced with good service records. The movement of No. 151 from the aiding at Bryn Athyn in violation of the train order cannot be explained.
