Board of Inquiry into 1912 Waterfall Wye derailment findings
The board of inquiry chaired by J L Palmer and including C J Husted R H Hunter and W Farrell investigated a derailment near Waterfall Wye in 1912. Conductor F Thor son engineer Ray Welker and others testified, a wheel from a 1904 American Car and Foundry axle failed due to an old flaw, the wreck involved C N W cars 77878 24652 and a barley car, and the commission ruled the accident unavoidable. The inquiry concluded the wheel failure could not have been detected by ordinary inspection and that the derailment occurred despite care at Kemmerer yard.
