Investigators say controller turned back before takeoff at crash site
Investigators say a single Lexington controller, momentarily focused on paperwork, allowed Comair Flight 5191 to taxi for takeoff from the shorter runway. The captain Jeffrey Clay steered onto the wrong runway, and the jet crashed, killing 49 people except first officer James Polehinke, who was hospitalized. FAA later acknowledged violating policy by using one controller for both tower and radar duties.
