ValuJet crash traced to a string of human errors
A May 11 shipment of oxygen generators to ValuJet 592 in Miami and later to Atlanta was identified as a contributing factor. SabreTech executives testified that faulty labeling, missing safety caps, and shop-floor confusion led to transporting hazardous oxygen canisters that overheated and ignited after takeoff, killing 110.
