Gas leaks blamed for Delta launch failures
A NASA Failure Review Board blamed gas leaks for malfunctions on Delta 85 and Delta 86 missions. Delta 86 failed after oxidizer vent valve leaks caused attitude disturbances and long coast thruster use, plunging into the Arctic. Delta 85 suffered hydraulic system leaks causing a near miss in orbit. OSO-7 later operated normally. The Delta program, managed by Goddard Space Flight Center, has 79 successes in 86 launches.
