A Social Security Administration survey shows many individuals lose benefits by neglecting to file for old age and survivors insurance. Clifford W. Swedlund, manager of the agency, notes gaps in outreach and claims awareness.

Virginia Mazuroo, two, died when a rear wheel fractured her skull as her father backed the truck near a barn yard in Rowley. The incident happened while she slept and wandered into the path of the vehicle.
