Apollo 13 tank wiring damage linked to takeoff
Apollo 13 investigators said wiring insulation inside the oxygen tank may have been damaged before the mission. Cortright, head of the Apollo 13 review board, cited drainage procedures that could have harmed wiring insulation during a prelaunch oxygen fill. He noted possible heater switch failures could have allowed 1,000 degree temperatures, risking insulation and triggering the explosion en route to the moon. Further tests and a final NASA report are planned for June 8.
