Care facilities cited for death abuse and med errors
Six Iowa care facilities faced state records for medication errors staff incompetence resident abuse and a death at Park View Homes Sioux City. Park View was fined 8000 after a three month inspection that found five federal and one state violation including a death by hypoproteinemia and untrained staff and an on-call nurse not licensed. Other facilities such as Fort Dodge Health and Rehabilitation Stacyville Community Nursing Home Hillcrest Decorah and Osage Rehab received fines from 500 to 13400 for issues including missed reporting abuse wrong medication insufficient staffing feeding risks and unsafe transfers. The cases illustrate ongoing compliance problems in Iowa care facilities and enforcement actions by state regulators.
