VA audit flags long waits and record falsification
The VA audit shows more than 57,000 veterans waited 90 days or more for initial appointments and 64,000 may have fallen through the cracks across 731 facilities. Thirteen percent of schedulers report being told to falsify schedules to shorten waits. The review spans the VA network amid fallout from Phoenix center concerns, with actions to reassign staff, hire more workers, and consider non-VA providers to accelerate care.

North Carolina and Virginia environmental officials join Duke Energy and the U S Fish and Wildlife Service in a cleanup agreement for the Dan River spill. Duke will cover reasonable cleanup costs and ongoing monitoring with no cap, and a similar pact was signed with the EPA last month. The spill coated 70 miles of river with coal ash, while Duke continues vacuuming ash from the river bottom and assessing future costs and potential penalties.
