Funeral Director Urged to Complete Legal Certificate

Officials stress proper completion and filing of the legal funeral certificate by funeral directors to ensure compliant handling of arrangements. Proper forms prevent delays and support accurate death record processing across agencies.

Funeral Home Assists in Completing Vital Statistics Form

After a death the funeral director compiles a statistical certificate to be signed by the attending physician and filed with the city clerk and the Department of Vital Statistics in Augusta. It requires birth place and date, father's and mother's names, military discharge papers if applicable, and the deceased's Social Security number for SSA reporting. A lump sum death benefit may be paid. We can record and keep this information on file for quick reference.
