SRS Incident Reporting Shows Underreporting To DOE
GAO found only 39 percent of Savannah River Site reactor incidents with significant hazard potential were reported to DOE from 1982 to 1987. Du Pont labeled 71 incidents as significant but filed 28 as unusual occurrences, with 29 of the rest meeting DOE criteria. DOE and Du Pont used different reporting systems leading to undercounting. DOE HQ ruled 36 of the 43 unreported incidents should have been unusual occurrences. SRS DOE staff counted 20.
