You must state (1) your name, mailing address and daytime phone number; (2) the permit number or the other recognizable reference to this application, (3) the statement ‘I/we request a public hearing", (4) a brief description ot how you, or the persons you represent, would be adversely affected by the granting of the application; (5) a description of the location of your property relative to the applicant's operations; and (6) your proposed adjustment to the application permit which would satisfy your concerns and cause you to withdraw your request for hearing. Before a permit can be renewed tor this facility, the applicant must demonstrate that all emission source are in compliance with all TNRCC air quality rules and regula tions and all applicable Federal regulation. ;
