ALTMAN New York Times Service NEW YORK By having JO men with heart disease drive through heavy morning traffic on a Los Angeles freeway, a team o[ doctors has found "compellingevilence" thai fitnall amounts of carbon mon- oxide from automobile exhaust aggravated their pre-existing cardiac condition for at least two hours thereafter. But when the same men breathed purified instead of pol- luted air while driving over the same freeway route, their con- dition called angina pectoris not worsen. Angina pec- toria, or angina for short, is a common heart condition that affects an estimated over 2 mil- lion North Americans. The California study has im- plications not just for heart dis- ease patients, other doctors said, but also for healthy peo- ple exposed to air pollution. It raised questions about what small amounts of carbon mon- oxide can do to healthy people and underscored the need for more research studies of the short term and long-term ef- fects of similar pollutants on people in everyday situations. Although many people have speculated about the damage produced by air pollution, the California medical investigation is one of the rare studies that has documented such dangers to the body's physiology in an everyday situation. "There's a lot of talk about pollution but when you look for physiologic s t u d i e s you just don't find said Dr. Wil- bert S. Aronow, a cardiologist at the Long Beach Veterans Administration hospital. "We all thought traffic jams must be harmful because peo- ple just don't feel well during traffic Aronow said. "But Is it really the team members asked be- fore designing their study. From a variety of tests on 10 male volunteers, aged 40 to 56 years, who had evidence or angina, the doctors document- ed serious physiologic abnor- malities. When these patients breathed the polluted freeway air, they developed raised levels of car- bon monoxide in their arterial blood, a slower heart rate, a lower blood pressure, decreased lung function, and a poorer ex- ercise performance. Three of the 10 patients had electrocar- diographic changes that show- ed the heart demanded more oxygen than could be delivered through the arterioscler otic narrowed coronary arteries. These results were obtained from an experiment in which the volunteers staged overnight in the hospital on two occasions. When the volunteers got up In the morning, the doctors measured their lung function from the air they exhaled, tha carbon monoxide, oxygen and carbon dioxide levels in blood drawn from an artery in their arm. The volunteers also exer- cised on a bicycle until tha crushing chest pains from an- gina began. At the same time, the doctors recorded the volun- teers' electrocardiograms, blood pressures and heart rates. Aronow said that although "the results probably would ap- ply anywhere traffic congestion further research was needed to prove this assump- tion. Results might be worse in cities where the traffic was stagnant rather than flowing more rapidly on a freeway. He went on: "Carbon monoxide Is very im- portant.
