VICTORIA (CP) British Columbia doctors appear to be heading for a crunch In their prolonged battle with the pro- i vincial government for a re- I vised medical care fee schedule. When a similar confrontation occurred in 1S70, the fee-tor- service doctors defused the situ- ation by declaring ft one-year moratorium on rate increases. The moratorium expired Jan. I, 1972, Now, after more than year of off-and-on negotiations with the Social Credit administra- tion, the province's physi- cians seem riolcrmined to force the issue. But their chances of squeezing rate increases from a cost-conscious government seem remote. Some doctors are calling for demonstration of "doctor power" while others speak of opting out ol the provincial medical care plan or compro- mising with a system of "bal- ance billing." Since B.C. first joined the fed- eral medical insurance system 196fl, (he doctors have had one tho rates they charge lor various services. REJECT OFFEH Earlier this year they re- jected a government offer cf a 10-per-cjit over-all increase In medical care costs and initiated a fee schedule averaging per cent above 1570 medical care rates. The medical services commission, which administers the B.C. medical care plan, con- tinued lo pay doctors 90 per cent or the fees listed in the B.C. Medical Association's 1970 fee schedule. In April, the UMClatioo re- vised January's billing schedule clown to fi.5 per cent lo conform to (he provincial government's wage increase guideline for pub- lic service employees. The government announced Tuesday that it rejected tlus re- vised fee rate and said it would make a counter offer shortly. Premier Yv. A. C. Bennett told the doctors last year they should freeze the fee rales until inflation is licked." It would appear the doctors have one of three choices: to accept the situation, adopt a system of balance billing or opt out of the medical care plan. Dr. Donald Shorting, a Victo- ria general practitioner, said In an interview the doctors will have no alternative but to bal- ance bills, a system he has been using. Under this system, Dr. Short- ing bills patients directly for the difference between 90 per cent of the 1970 tee schedule and 90 per cent of the proposed 1972 schedule. Under the Medical Services Act, a doctor who undertakes balance billing must first notify patients he intends to charge above the medical care plan schedule, and they must ac- knowledge and agree to the ar- rangement. The extra billing amounts to 45 cents for an office visit, ?2.70 for a home visit, for caro throughout maternity, and SB for most operations.
